Percutaneous Transtracheal Aspiration in the Diagnosis of the Various pulmonary diseases

Abstract
The usefulness of anaerobic culture of percutaneous transtracheal aspirates was evaluated in 21 patients with various pulmonary conditions who were admitted to the Department of Chest Medicine of the National Medical Center from Feb. 1981 to Nov. 1981. We compared the aerobic and anaerobic culture of percutaneous transtracheal aspirates with the culture of expectorated sputum.
1. Bacteria were recovered in 4 patients by the culture of percutaneous transtracheal aspirates. Of these 4 cases, bacteria different from those isolated from the culture of sputum were recovered in 3 cases: anaerobes were recovered in only 1 case and sputum culture was negative in 1 case. Therefore we think it possible to differentiate anaerobes in the lower respiratory tract from normal flora in the upper airway in the case of lower respiratory tract infections.
2. Characteristics of sputum and transtracheal aspirates (mucoid or purulent), use of antibiotics, techniques of TTA, use of transport media, and anaerobic laboratory techniques are believed to affect the result of percutaneous transtracheal aspiration.
3. After the procedure of percutaneous transtracheal aspiration, only 2 patients developed mild subcutaneous emphysema, which subsided spontaneously several days later.
